What you’ll do

Are you passionate about delivering exceptional care services? We are seeking a dedicated and skilled Field Care Supervisor to play a key role in ensuring the highest quality of care for our clients. As a vital member of our team, you will engage in various responsibilities aimed at supporting and enhancing our care support services.

Key Responsibilities:

  • Assessment and Review: Conduct thorough assessments and reviews of individual services to ensure they meet the highest standards of quality and effectiveness.
  • Supervision and Support: Provide practical supervision, instruction, and support to our team of care workers, fostering a positive and collaborative work environment.
  • Rota Planning: Collaborate with care coordinators to develop realistic and effective care worker rotas, taking into consideration the unique needs of each individual and the specific skills of our care workers.
  • Needs and Risk Assessments: Conduct regular reviews of individual services, including repeated and renewed needs and risk assessments, ensuring our care plans remain current and relevant.
  • Multi-disciplinary Collaboration: Participate in multi-disciplinary reviews of service users' needs, contributing valuable insights and expertise to the team.
  • Competency Assurance: Shadow new care workers in the field to ensure their competence and provide on-site observation and evaluation of care worker practice as part of routine supervision or in response to specific performance concerns.
  • Community Engagement: Spend the majority of working time in the community, actively engaging in the delivery of care support services, while also contributing to office-based administrative tasks as needed.

Qualifications


What you need

Qualifications and Experience:

  • Risk and Needs Assessments: You will demonstrate proficiency in conducting comprehensive initial risk and needs assessments for individual service users. This includes assessments conducted in various settings, such as the clients home, hospitals, or residential/nursing homes.

  • Collaborative Approach: Work collaboratively with clients, their families, carers, advocates, healthcare professionals, and other stakeholders to facilitate the seamless delivery of holistic packages of individualised care support.

  • Client-Centered Engagement: Prioritise a client-centered approach, ensuring that the unique needs, preferences, and goals of each service user are considered and incorporated into the care planning process.

  • Adaptability: Display flexibility in adapting assessment methodologies to different settings, demonstrating sensitivity to the diverse environments where assessments may take place.

  • Communication Skills: Possess excellent communication skills to engage effectively with a wide range of stakeholders, fostering an environment of open dialogue and collaboration.

  • Holistic Care Perspective: Embrace a holistic perspective in care delivery, recognising the interconnectedness of physical, emotional, and social aspects of well-being when assessing and addressing the needs of service users.

  • Compliance: Stay informed about and adhere to relevant regulatory guidelines and standards related to conducting risk and needs assessments, ensuring the highest standards of care and compliance.
